INCIDENT REPORT
ADOPTED: JUNE 29, 1995

The completion of an Incident Report form is required for any accident, bomb threat, death or severe medical problem, drug or psychiatric problem, earthquake, fire and explosion, power failure, robbery, tornado, or bomb threat emergencies.
